But lab testing can be valuable before symptoms become disruptive.
A comprehensive baseline gives you and your healthcare provider a snapshot of where you are today. Future testing can then help identify changes, monitor treatment, and provide a more complete picture of your health over time.
So, when should you start getting annual labs? And what is the difference between routine screening, baseline labs, and more comprehensive optimization testing?
What Age Should You Start Getting Annual Labs?
There is no single age when every healthy adult suddenly needs the same annual blood panel.
Recommended screening depends on your age, medical history, family history, medications, symptoms, and individual risk factors. For example, the U.S. Preventive Services Task Force recommends diabetes screening for adults ages 35 to 70 who have overweight or obesity, while other preventive screenings follow different age and risk-based schedules.
That is different from choosing to establish a comprehensive baseline.
For many adults, getting baseline labs in their 20s or 30s can provide useful information that can be compared with future results. As you move through your 30s, 40s, 50s, and beyond, changes in metabolism, hormones, thyroid function, nutrient status, and cardiovascular risk may become increasingly relevant.
The goal is not simply to ask:
“Is this result inside the normal range?”
A better question may be:
“Has something meaningfully changed from my previous results, and does that change fit with how I am feeling?”

Baseline Labs vs. Annual Optimization Labs
These terms are sometimes used interchangeably, but they serve different purposes.
Baseline Labs
Baseline testing establishes a starting point. Depending on the individual and the panel ordered, testing may evaluate areas such as:
- Blood sugar and insulin
- Cholesterol and triglycerides
- Liver and kidney function
- Thyroid function
- Iron and ferritin
- Vitamin B12, folate, and vitamin D
- Inflammation markers
- Sex hormones
- Other markers based on symptoms and health goals
A baseline becomes more valuable over time because future results can be compared with your own previous data.
Annual Optimization Labs
“Optimization labs” is not a standardized medical guideline or a single universal panel. It is a term often used for more comprehensive testing designed to help a provider look beyond a limited set of routine screening markers.
For someone working on weight management, hormone optimization, energy, hair health, metabolic health, or healthy aging, a broader panel may help provide additional clinical context.
Importantly, more testing is not automatically better. Lab results should be selected and interpreted based on the individual, rather than treated as a checklist or used to diagnose a condition in isolation.
When Should You Check Your Hormones?
There is no universal schedule for hormone testing.
Hormone labs may be appropriate when you are experiencing symptoms such as:
- Persistent fatigue
- Low libido
- Changes in sexual function
- Hot flashes or night sweats
- Changes in menstrual cycles
- Difficulty maintaining muscle
- Unexplained body composition changes
- Mood or sleep changes
- Symptoms that may suggest low testosterone or another hormonal concern
For patients already receiving hormone replacement therapy, laboratory monitoring may be used alongside symptoms, treatment response, side effects, and other clinical factors.
For men receiving testosterone therapy, the Endocrine Society recommends appropriate follow-up monitoring, including testosterone and safety-related assessments based on the individual patient.
For women, hormone testing can be more complex because hormone levels may fluctuate significantly during perimenopause and across the menstrual cycle. A single laboratory result does not always tell the whole story. Symptoms, medical history, age, menstrual patterns, and other clinical factors remain important when evaluating whether hormone therapy may be appropriate.
Losing Your Hair? Don’t Forget to Check Ferritin
When people notice hair shedding or thinning, they often immediately reach for shampoos, supplements, or topical treatments.
But sometimes the first place to look is inside the body.
Ferritin is a blood marker that reflects the body’s stored iron. Low ferritin may be associated with certain types of diffuse hair shedding, including telogen effluvium, although hair loss has many possible causes and the research does not support using ferritin alone to explain every case of hair loss.
That distinction matters.
Hair loss may also be related to:
- Hormonal changes
- Thyroid dysfunction
- Significant weight loss
- Nutritional deficiencies
- Illness or physical stress
- Medications
- Genetics
- Menopause or postpartum changes
A comprehensive evaluation may include ferritin alongside iron, thyroid markers, vitamin D, B12, hormones, and other tests selected by your provider.
Do not start taking iron simply because you are losing hair. Iron supplementation should be based on appropriate testing and medical guidance.

This type of panel can provide a much broader picture than looking at a single number in isolation.
Preparing for Your Lab Appointment
This is a fasting lab panel. Patients should follow the fasting instructions provided by Valley Medical before their blood draw, including instructions regarding food, beverages, medications, and supplements.
One additional consideration: biotin supplements can interfere with some laboratory tests, particularly thyroid testing. The American Thyroid Association advises stopping biotin before thyroid blood testing, but patients should confirm the appropriate timing with their healthcare provider.
Your Lab Results Are Data. The Interpretation Is What Matters.
A lab report can contain dozens of numbers, but numbers without context have limitations.
A result may fall within a laboratory reference range and still need to be considered alongside your symptoms, medical history, medications, previous results, and overall health. Likewise, an abnormal result does not automatically mean you have a specific medical condition.
That is why comprehensive testing is most useful when it is part of a larger conversation with a qualified healthcare provider.
